    Notice for Self-pay COVID-19 Test

                                                            

    Date of issuance:  June 29, 2021

    A. Time of application and sample collection:

        *From Monday to Sunday: application services for self-pay COVID-19 test is available.

    *Urgent case:

    From Monday to Sunday: please complete your registration and sample collection (test) respectively before 09:30 and 10:00 in the morning. You may pick up the report after 19:30 in the evening on the same day.

    *Non-urgent case:

        Specimen collection (test) is available Monday through Sunday all day. After completing the specimen collection, you may pick up the test report after 19:30 in the evening on the following day (same applies to holidays).

      *Both urgent and non-urgent cases are collected at ER. Though the service is provided on demand, you may still need to line up and wait for the test to be taken according to the actual on-site situation at ER. We apologize for any inconvenience caused.

     

    B. Check-in (application) location:

    *The office hours are Monday through Friday from 08:00 to17:20 and from 08:00 to 11:40 on Saturdays. Please wear a surgical mask during the whole time and bring the inspection proof documents to the service counter on the first floor for verification. Please bring the inspection proof documents for verification at ER registration outside office hours.

    *Charging standard:(since June 29 2021)

         The charge is 1. NTD 4400 for an urgent case and 2. NTD 3400 for a non-urgent case.

         3. The report will only include the result of the test. If a certificate of diagnosis is required, the fee is NTD 250 for each copy of the Chinese and English version certificate. Additional NTD 20 will be charged per extra copies.

         * For those who require a certificate of diagnosis: please provide your personal information (name and address) in English when filing the application for our hospital is not responsible for the translation of your personal information.

     

     

     

    C. On-site inspection documents:

        *Documents required

         1. Application Form for Self-pay COVID-19 (Coronavirus) Test provided by Taiwan Centers for Disease Control

         2. The corresponding supporting documents (such as the hard copy of both the original copy and copy of electronic ticket, proof of purchase, ticket booking record, etc.) provided by the applicable applicant.

         3. One original copy and one copy of foreign passport.

         4. One original copy and one copy of Identification Card.

     

    D. Applicable to:

      1.People who are at home isolation/quarantine that need to visit other places owing to emergency special factors, such as accidents or serious illnesses of relatives: a. application form; b. documents related to the reasons for application.

      2.People who enter other countries/regions owing to emergency special factors, such as accidents or serious illnesses of relatives abroad and require a medical certificate: a. application form; b. documents related to the reasons for application, such as electronic ticket, proof of purchase, ticket booking record, etc.

      3.People who need the medical certificate for work-related reasons: a. application form; b. work proof documents: employee ID, work visa, business trip notice, electronic ticket, proof of purchase, ticket booking record, etc., for instance.

      4.People who have short-term business needs: a. application form; b. documents for short-term businessmen, e.g., itinerary in Taiwan, or epidemic prevention plan.

      5.People who need the medical certificate to study abroad: a. application form; b. proof of enrollment: student ID, student visa, letter of admission, electronic ticket, proof of purchase, ticket booking record, etc., for instance.

      6.Departure from foreign countries or people from China, Hong Kong, and Macau: a. application form; b. foreign passport, entry permit, electronic ticket, proof of purchase, ticket booking record, etc.

      7.Family members of relevant departure applicable people: a. application form; b. Identification Card and the relevant proof of relationship with the departure applicable people, e.g., the household certificate, certificate of student status and work proof documents of the applicable people.

      8.People who have approved by the Central Epidemic Command Center (CECC) for Severe Special Infectious Pneumonia: a. application form; b. consent letter for severe special infectious pneumonia from CECC.

      9.People who need the test due to other reasons: a. application form.

     

    E. Pickup location and service hours:

        *Urgent case: the report will be ready to pick up on the same day after 19:30.

        *Non-urgent case: you may collect the test report on the following day after 19:30.

        *Remark:

          1. Please provide a copy of the application documents. The applicant (examinate) is responsible for the time delay incurred as a result of incomplete information provided.

          2. Should there be any adjustments or changes in the content of this announcement, please refer to the on-site announcement for actual situation.

            *pickup location of the report: registration and cashier counter at ER

            *Pickup time: 24 hours

            *If any abnormalities are found during the inspection process, the issuance of the test report will be postponed as the specimen will be reconfirmed and verify again. The pickup time should be based on the actual completion time of the hard copy of the report.

     

    F. Documents to collect the report

        *Pickup in person: 1. original outpatient receipt    

    2. original copy of ID card (or NHI card or driver's license)

        * Entrustment: 1. original outpatient receipt     

    2. applicant’s original copy of ID card (or NHI card or driver's license)

        3. trustee’s original ID card (or NHI card or driver's license)     

        4. letter of Authorization

        *Notice: when collecting the test report or certificate of diagnosis, please make sure that the personal information and name shown in both Chinese and English, etc. are correct before signing the affidavit for collection. We hereby declare that our hospital will not bear any responsibility for any problems or losses caused by the typographical error of data after pickup.
